Key information about this project: 
The purpose of this study is to examine the relationship between personality and academic success in children aged 6-17 years old following a hospitalization of 7 days or longer. Caregivers of children who have been recently hospitalized will be asked to answer questions about their child’s personality, academic performance, overall adjustment, and their family system. We expect this to take about 30 minutes.

Why am I being asked to take part in this project? 
We are asking you to participate in this project because you are a United States resident over the age of 18, you are fluent in English, and you are the legal guardian of a 6-17-year-old child who has been discharged from a hospitalization of 7 days or longer who has re-entered the school setting within the last 2 years.

What should I know about this research study? Whether or not you take part is up to you. You can choose not to take part. You can agree to take part and change your mind at any time.  Your decision will not be held against you or have any consequences.  You can ask all the questions you want before you decide to participate.   You can skip any questions you do not wish to answer during the study. There is no compensation for taking part in this study. Your personal information will be kept confidential (private).

An Institutional Review Board (IRB) has reviewed and approved this project.

How long will the research last? 
We estimate your total participation will last about 30 minutes.

How many people will be studied? 
We expect about 300 people to participate in this study.  

What happens if I say “Yes, I want to be in this research”? 
You will be asked to complete an online questionnaire about your child’s personality, academic performance, overall adjustment, and your family system.

What happens if I do not want to be in this research? 
You may choose not to participate and there will be no consequences. 

What happens if I say “Yes”, but I change my mind later?
You can leave the research at any time, and it will not be held against you.  
If you begin the online survey, you can choose to withdraw by simply exiting out of the browser at any time. If you withdraw, no more information will be collected from you.

Is there any way being in this study could be bad for me? 
The risks associated with this survey are not higher than those of daily living. There is no known risk of discomfort. You can skip any questions you do not wish to answer or exit the form at any point.  In addition, while we do everything we can to protect the confidentiality of all research data, a breach of confidentiality is a potential risk.

What happens to the information collected for the research? 
The consent form and survey are being hosted on Qualtrics software which involves a secure connection. More information about this may be viewed at https://www.csuohio.edu/research/qualtrics. All electronic information will be stored on a password-protected computer only usable by the research team. Your identity will not be linked with your data. Any resulting research products will not include personally identifying information of any kind, including names.  

Will my information be kept confidential? 
Members of the research team who may see your information will maintain your confidentiality (privacy) to the extent of laws and university policies. No identifiable information will be published or presented.  

Despite our best efforts, we cannot guarantee complete secrecy. Organizations that may inspect and copy your information include the IRB and other representatives of CSU.  

Additionally, identifiers might be removed from any information you provide. After such removal, the information could be used for future research studies or shared with other researchers without seeking additional informed consent from participants.
